The Outcome and Assessment Information Set (OASIS) is a standardized data collection tool used by home health agencies (HHAs) to assess patient health status, track outcomes, and ensure quality care. As part of ongoing efforts to enhance patient-centered care and align with value-based payment models, the Centers for Medicare & Medicaid Services (CMS) introduced OASIS-E1, which took effect on January 1, 2025.

Key Changes in OASIS-E1 

  • Refinements to Data Collection

    • Four key changes occurred in OASIS-E1 revisions, including removing episode timing, discharge goal setting, and therapy need counting. This means assessments are more streamlined and care planning is focused for immediate, person-centered care. Covid-19 vaccination status was added to help track patient safety and determine public health standard adherence.
  • Minor Revisions to Instructions and Wording:

  • Standardization for Interoperability and Data Sharing

    • Remember that all of the OASIS updates are meant to increase communication of patient health records across healthcare systems. Particularly in post-acute care, learn to welcome the process of ongoing adaptations from CMS, so we can all collect similar patient data and use it to create a culture of person-centered care and positive outcomes.

A Few Tips on Implementing OASIS-E1 

1. Conduct Staff Training: Ensure clinicians and administrators are well-versed in the changes to avoid documentation errors.
